Child care providers can use this form to inform the Parent Services Department at the Child Care Council of vacancies, tuition changes, and hours of operation.

 

Child Care Referral Questionairre

Child Care Providers must complete this form two times each year to update records on their program, environment, fees, openings, and more.

Parent Child Care Referral Follow-up

Input from parents who have used our referral service provides us with feedback pertaining to their search for child care. The information enables us to improve our level of service and let us know if any changes are necessary.

Any information that is provided to the Capital District Child Care Council is kept confidential and will not be shared with any third party without written permission. The Capital District Child Care Council is committed to maintaining the confidentiality of its consumers.
